5.1.1. Any person under 18 years old needs the express consent of their parents or guardian before submitting their personal data to Global Health Company via the Website. Global Health Company shall assume that if the minor accesses the Website is under given authorization of their parents, guardian or legal representative.
5.2. Information Request Forms
5.2.1. All the Information Request Forms will be handled, at first, by our Medical Consultant Center (from now on mentioned as MCC). The personnel which conforms such department is trained to answer general questions about the procedures and terms of the service. If the patient has questions related to medical issues, the consultant will pass the questions on to the professional and the answers back to the patient. They act as a link between the patients and the physicians.
5.2.2. The patient can ask for a conference call with the physician, the MCC representative will arrange the conference call according to the physician's schedule.
5.2.3. All the exchanged information between users and MCC is free of charge and it does not represent any kind of agreement between them.
5.3.1. All the fee services and procedures will be quoted by MCC. The quotation is subject to changes depending on the outcomes of the pre-surgical tests and the physicians first consultation. This could happen for many reasons, such as preexisting diseases, non suitability for the treatment chosen by the patient, etc.
5.3.2. The quote will be valid for a period of 30 days, after that, the patient/user will need to ask for a new quote.

5.4.1. To book a medical trip, it will be necessary for the patient to make a deposit of the 20% (twenty percent) of the total amount of the service quoted by MCC. The remaining balance needs to be paid upon arrival prior to the surgery date.
5.4.2. This deposit will guarantee the time frame chosen between the patient and the MCC consultant.
5.4.3. The same deposit will freeze the price for a period of six month, after that, the service can be re quoted and the deposit assumed as part of the proper reservation deposit, having the patient to complete the necessary amount to cover the 20% deposit.
5.4.4. If the patient wishes to maintain the price for a longer period than 3 (three) months, the patient will need to pay the total amount of the service before the six month expires.
5.4.5. Once the medical trip is booked the patient will be assisted by one of our Medical Care Department's representative (from now on MCD representative), who will be responsible for the patients agenda.
The patient will be asked for his/her clinical records and related information according to every case, in order to assure that the patient is in perfect conditions to receive the chosen intervention, or to take the necessary precautions in case the patients has a preexisting disease.
5.5.2. Global Health Company reserves the right of asking for any additional information regarding the patient's health condition, to the patient and/or the patient's physician.
5.5.3. All the requested interventions may be subject to changes according to the diagnosis and the discretion of the physician. It all depends on the physician's first evaluation, taken place in our facilities.
5.5.4. Patients with preexisting conditions will be asked for a series of extra exams that are not included in the pre-surgical examinations. These extra exams are going to be charged according to every case.
5.5.5. The MCD representative will provide to the patient with a specific date for the pre-surgical tests, these are mandatory for any surgical intervention.
